My Take

The pdf is an excellent info-packed, must-see presentation / book / slide set!

I’m grateful to Dr. Holthorf for his pioneering work, incredible research and helping bring these critical peptides for health & wellness to market.

In the next section you’ll find a summary of the full book. It’s important to remember that his guidance is general, not based on unique differences and to be tailored as needed.

Here are just a few notes from me:

Key points of agreement

Short-chain, organ-targeted bioregulators are powerful, gentle, and can reset tissues toward healthier function.

They influence gene expression and tend to be well tolerated, including in combos.

Immune, pineal, and vascular support are critical for energy, sleep/circadian rhythm, brain function, and aging.

Thyroid, pancreas, cartilage/skin, gastric mucin and other bioregulators should be layered based on symptoms and goals.

TB4 Frag, KPV, and brain blends like CogniPep/CerebroPep are strong additions for repair, inflammation, and cognition.

Safety lens always favors short-chain orals and gentle combos; we too avoid injectables and anything that pushes rather than modulates.

Where our approach differs from the book

1. BPC 157 is not in my protocols

Some people do fine with it, but I’ve seen anxiety, flat mood, and mast cell flares reported quite often in our group and others, so I can’t personally recommend it.

I prefer TB4 Frag for broad tissue repair plus KPV for inflammation and gut.

2. We don’t have a ‘core trio’

Symptoms always guide order and to be fair, Dr Holthorf doesn’t like giving protocols because everyone is unique. What he suggests in the book is not tailored advice.

Some common firsts we see working well are NAD+ and sometimes Plavelle to steady energy and prepare sensitive users for possible KPV die off.

3. Our dosing is much less

I favor small, frequent amounts because peptides signal briefly (when beginning, before genetic changes take hold) and work at very low amounts (per Dr. Holthorf’s research).

Note: Dosing we give is always approximate, adjusted by feel.

4. Gut barrier first when needed

We often pair KPV with Larazotide, and add Gastric Mucin (Gastrogen) for surface protection if reflux or irritation is part of the picture.

5. Immune nuance

Thymogen is used situationally, especially during acute illness, not as a constant baseline for everyone. Our acute illness protocol is on the dosing page

What Are Oral Peptide Bioregulators and Why Do They Matter for Your Health?

Peptide bioregulators are super short chains of amino acids (just 2-4 long, unlike longer peptides or proteins). They’re like little messengers that fine-tune your body’s processes, acting as hormones, immune boosters, or metabolic regulators.

The oral versions are easy to take (no injections) and target specific tissues to “reset” them toward a younger, healthier state.

The big idea here is that aging and chronic illnesses often stem from a weakening immune system.

As we get older (starting around age 15-30), the thymus gland (key for immune balance) shrinks, leading to an “immune shift” that causes inflammation, infections, autoimmunity, fatigue, and multi-system problems.

Pineal and vascular function also decline, messing with hormones, sleep, and blood flow.

Bioregulators help reverse this by boosting immune modulation—basically, rebalancing your body’s defenses so you can fight off issues better and feel more vibrant.

Key takeaway for improving health: Start with the “core trio” (thymus, pineal, vascular bioregulators) as a foundation. They’re like the base of a smoothie—add them first, then layer on tissue-specific ones based on your needs (e.g., thyroid for energy, brain for focus).

This can help with anti-aging, energy, immunity, and chronic conditions like fatigue, pain, or hormone imbalances.

Core and Specific Bioregulators: Benefits and How to Use Them

Here’s a breakdown of the main ones he discussed, with benefits focused on what he suggests you can do to feel better.

Dosages – typically 1-2 capsules a day for 2-3 months, but he recommends to consult a pro for personalization. They’re safe, with no reported side effects in the trials.

His Core Trio

1. Thymic (Thymus) Bioregulator (e.g., Thymogen Alpha-1: contains Vilon, Thymulin)

Benefits: Reverses immune decline, boosts NK cells, fixes CD4/CD8 ratio (>2.5 is ideal for healthspan), reduces inflammation, fights infections/autoimmunity, improves energy, mood, and longevity.

Great for chronic fatigue, infections, or age-related weakness.

What to do: If you’re feeling run-down or have immune issues, this is a must. It can cut cancer risk, speed healing, and protect against stress.

Dosage/Protocol: 1-2 caps/day for 3 months. Combine with Thymosin Alpha-1 alternatives for extra potency (100x stronger than injectables in some studies).

Combinations: Pair with TB4 Frag or KPV for anti-inflammation and healing.

2. Pineal Bioregulator (e.g., PinealPep Epitalon)

Benefits: Normalizes melatonin for better sleep, reverses infertility, lengthens telomeres (anti-aging at the DNA level), reduces morbidity/mortality from cancer/CVD, improves hormone balance, brain function, and stress handling.

What to do: If sleep, mood, or hormone issues are your thing, this helps reset your body’s clock. Studies show it extends lifespan and healthspan.

Dosage/Protocol: 1 cap every 3 days for 4 months, or 1/day for 2 months in combos.

Combinations: With ovarian/testes for fertility, or brain/vascular for cognition and heart health.

3. Vascular Bioregulator (e.g., Vesugen, Ventfort, KED) Also part of: CogniPep & PinealPep

Benefits: Repairs blood vessels, reduces atherosclerosis/stroke risk, normalizes cholesterol/endothelin-1, boosts nitric oxide for better flow, fights inflammation/SASP, improves endothelial function and elasticity.

What to do: For heart health, circulation, or brain fog (better blood flow = better oxygen). It can reverse vascular aging and lower CV risk.

Dosage/Protocol: 2 caps/day for 3 months.

Combinations: With pineal/brain/liver for atherosclerosis or diabetes (one study showed 3x mortality reduction in heart attacks).

Some Other Tissue-Specific Bioregulators (Add Based on Needs)

Thyroid/Thyroid Axis (e.g., ThyroPep: contains TABa-C44, Alpha-24C)

Benefits: Stabilizes thyroid function, reverses hypo/hyperthyroidism and autoimmunity (Hashimoto’s/Graves’), boosts T3 levels (up 300% in studies), improves energy, weight loss, mood, metabolism, and longevity. Reduces nodules and mitochondrial issues.

What to do: If you’re tired, gaining weight, or have “normal” labs but feel off, test beyond TSH (e.g., free T3, rT3, Thyroflex for reflex time). Can normalize in 20 days.

Dosage/Protocol: 1-2 caps 2x/day for 3-4 months. Better than T4 alone for T3 boost.

Brain/Subcortical (e.g., CogniPep, CerebroPep, Pinealon, Cerebrolysin oral)

Benefits: Improves cognition, memory, headaches, dizziness, emotional stability, EEG stability. Treats CNS disorders, stroke recovery, and atherosclerosis.

What to do: For brain fog, anxiety, or neuro issues—reduces symptoms by 2-3x in studies.

Dosage/Protocol: 2 caps/day for 3 months.

My note: Watch for these Easter eggs (not yet released product mentions!)

Pancreas (e.g., Suprefort, Pancragen)

Benefits: Regulates blood sugar, prevents/reverses diabetes, improves lipid metabolism, reduces body fat, boosts endurance.

What to do: For blood sugar control or metabolic syndrome—longer-lasting than standard treatments.

Dosage/Protocol: 2 caps/day for 3 months.

Muscle (e.g., Gotratix)

Benefits: Boosts strength, endurance, recovery; reduces fatigue; improves aging biomarkers.

What to do: For better workouts or muscle weakness—faster recovery and less soreness.

Dosage/Protocol: 2 caps/day for 2 months.

Prostate (e.g., Testovive, Testagen)

Benefits: Reduces pain/inflammation in prostatitis/BPH, improves urination, libido, sexual function, sperm quality.

What to do: For men’s prostate issues—64% pain-free in 30 days.

Dosage/Protocol: 1 cap 2x/day for 30 days.

Testes (for “Low-T”)

Benefits: Boosts testosterone (70%), LH/FSH, libido, mood, energy; reduces fatigue, sweating, headaches.

What to do: For low energy or hormone imbalance in men.

Dosage/Protocol: 1 cap 2x/day for 30 days.

Ovarian (with Pineal) eg. Ovalis

Benefits: Improves fertility, hormone balance, reduces menopause symptoms; boosts FSH/LH/estradiol/AMH.

What to do: For women’s hormone/fertility issues—helps pregnancy in age-related decline.

Dosage/Protocol: Ovarian 2 caps 2x/day + pineal 1 cap every 3 days for 4 months.

Skin/Dermatologic (Collacell)

Benefits: Rejuvenates skin, hair, nails; anti-aging effects.

What to do: Topical or oral for youthful appearance.

Other mentioned: Liver (Liverra, for metabolism/NASH), Kidney (Renolux) Cartilage/bone (Cartesyl – joint health), Adrenal (Adrenact – stress).

• Additional Key Oral Peptides mentioned in the book (Not Bioregulators)

These are peptides that complement the bioregulators, especially for inflammation, healing, and brain/gut support.

They’re oral, stable, and often combined with the core trio for chronic issues.

Dosages from studies/cases: typically 1-2 caps or sprays 2-3x/day for 1-3 months—safe with no side effects noted.

1. TB4 Frag (Thymosin Beta-4 Fragment, e.g., TB4 Active Frag Max)

Benefits: Potent anti-fibrotic (10x stronger than full TB4), boosts immune modulation, removes senescent cells, reverses kidney damage/fibrosis in diabetes, promotes hippocampal neurogenesis for better memory/spatial awareness. Key for chronic illnesses, inflammation, and tissue repair.

What to do: If you have fibrosis, kidney issues, brain fog, or need extra immune/anti-aging support, this is a game-changer. The “capped” natural isomer (acetylated/amidated) is more potent and bioavailable.

Dosage/Protocol: 2 caps 2x/day (bid) for 2-3 months. Includes thymus peptide lysate for synergy.

Combinations: With thymic bioregulators for senescent cell removal; BPC-157/KPV for gut/heart; in cases, boosted energy, endurance, and recovery in CFS/heart failure.

2. KPV

Benefits: Anti-inflammatory/antimicrobial (beats fluconazole for Candida), improves leaky gut by boosting transepithelial resistance/tight junctions, reduces cytokines/allergens, induces tolerance, protects brain via vagus nerve/cholinergic effects. Great for IBD, infections, allergies, and stress-induced inflammation.

What to do: For gut issues, chronic inflammation, or autoimmune flares—it’s stable, targets tissues well, and makes resistant bacteria susceptible to antibiotics again.

Dosage/Protocol: 3 sprays (nasal/oral) 3x/day (tid) or 1-2 caps bid for 1-3 months. Natural isomer (acetylated/amidated) is best.

Combinations: With BPC-157/TB4 Frag for POTS/racing heart; thymic for overall immunity; in cases, eased GI discomfort and anxiety.

Note: Our KPV page with our specific dosing is here.

3. BPC-157 (Body Protection Compound-157)

Benefits: Heals GI tract (IBD/Crohn’s/MS models), repairs neurologic damage (brain/spinal/peripheral), boosts bone/tendon/ligament recovery, fixes urinary incontinence, counters toxins (e.g., alcohol, burns), supports heart (HTN/POTS/CHF), microbiome, and gut-brain axis. Antioxidant, epigenetic modulator.

What to do: For gut disorders, injuries, neuro issues, or toxin exposure—stable oral form (Ac-BPC-157-NH2) absorbs intact, no need for enhancers.

Dosage/Protocol: 2 caps bid for 1-3 months (oral/intragastric/nasal/transbuccal/transdermal all work).

Combinations: With TB4 Frag/KPV for POTS/CFS; thymic for immunity; in cases, improved sleep, pain, and energy in IBS/Crohn’s.

4. CerebroPep (Oral Cerebrolysin) also now, CogniPep

Benefits: Brain immune modulator, anti-inflammatory, improves cognition, bioelectrical activity, EEG stability; treats cognitive decline, brain injuries, toxic encephalopathies.

What to do: For memory, focus, or neuro recovery—single dose enhances brain function quickly.

Dosage/Protocol: 1-2 caps/day for 1-3 months (oral version of injectable Cerebrolysin).

Combinations: With pineal/brain bioregulators for anti-aging; BPC-157 for gut-brain; in cases, sharpened depth perception, writing, and pressure-handling in CFS/TBI.

Dr Holthorf’s Bioregulator Protocols, Combinations, and Integration

Note: All 27 of the Bioregulator peptides and how we use them are covered in our Bioregulator Guide, here.

His General Protocol: Take 1-2 caps/day for 2-4 months, often cycling (e.g., 10 days/month for anti-aging). Rotate 5-7 bioregulators for broad effects, like in the TAP study (reversed telomere age by 7 years/year).

Combinations for Power:

Anti-aging/longevity: Thymus + pineal + vascular + brain + liver (e.g., for atherosclerosis—reduced cholesterol 30%, triglycerides 40%).

Diabetes/metabolic: Pancreas + subcortical brain + pineal (better glucose control than standard therapy).

Heart/CV: Vascular + brain + pineal + liver (3x lower mortality in MI).

Hormone issues: Thyroid axis + testes/ovarian + pineal.

Immune/chronic illness: Thymus + TB4 Frag + KPV (for inflammation, infections).

Dr Holthorf’s Integration Tips

• Acceptable to combine with other therapies like ozone, IV nutrients, LDN, probiotics, binders, or stem cells. For chronic cases, add antibiotics if infections (e.g., Lyme). Avoid if pregnant/breastfeeding without doc approval.

Limitations and Monitoring

Limitations: Safe in studies—no side effects, addictions, or complications. But results vary.

They do not require FDA-approval as drugs (they are supplements), so quality matters – stick to reputable sources.

Monitoring: Track symptoms (energy, mood, pain). Blood tests: CD4/CD8 (>2.5 ideal), TSH/free T3/rT3, telomere length/EpiAge, inflammation (CRP, hTGFb, C4a), NK cells, coagulation markers. Retest every 1-3 months. Use tools like Thyroflex (reflex time) or RMR for thyroid function.
